Physical Exam Requirements
New York State Education Law requires health examination for all students entering the district and those entering Pre-K or Kindergarten, 1st, 3rd, 5th, 7th, 9th, 11th grades. A dental certificate which states your child has been seen by a dentist or dental hygienist is also asked for at the same time.
Students playing interscholastic sports are required to have an annual physical exam. Athletic participation clearance is based on the review of a student's current physical and health history.
Students requesting working papers are required to have a physical completed within the past year.
Health examination forms may be faxed, emailed or they can be brought to the school during business hours. These required physical exam forms are to be turned into the Health Office within the first 30 days from the start of the school year or upon entry to the Clyde-Savannah Central School District.
Physicals are good for one year from date of completion.
Physicals must be completed by a NYS licensed provider.
Physicals must be completed on the NYS School Health Examination Form.
It is always best for student's own Primary Care Physician to complete the health examination for consistency with their health history. Please schedule an appointment for this to be completed by their PCP. If we do not receive the health examination form, your child will be scheduled for an exam at school with our district provider
