Emergency Care Plans

Students with a history of chronic illness or concerning conditions such as allergies causing Anaphylaxis, Asthma, Seizures, or Diabetes require Emergency Care Plans on file each school year. 

To provide the appropriate health care while the student is at school, these care plans should be completed by the student's doctor and submitted to the school Health Office prior to the beginning of the school year. 

These care plans will be shared with the appropriate staff unless otherwise requested. Please also have a current written doctor's order for any medication they may need to treat this condition while at school. See medication tab for more information.
